            After yet another school shooting in the United States left 19 children dead, many parents in grief, and a multitude of survivors in agony, it is time we recognize the impact that is left on the survivors of such devastating events. Although children are known to be particularly resilient, such impactful experiences may lead to post-traumatic stress disorder (PTSD).

            In children, PTSD symptoms are often overlooked or confused with other psychiatric disorders. These symptoms may include restlessness, fidgety behavior, trouble focusing or staying organized, reliving traumatic events through thought or play, nightmares, inconsistent sleep patterns, intense fear or sadness, angry outbursts, avoiding topics associated with the traumatic event, and feelings of denial. These emotions may be triggered by something that reminds the child of the event.

            Such devastating events are hard for anyone to understand, especially a child. If your child has experienced trauma, the symptoms can be best treated if they get in contact with a mental healthcare provider as soon as possible. Even if the child is not ready to talk about the events, many psychologists and psychotherapists can understand the situation by observing the child’s behaviors. Multi modal psychotherapy including cognitive behavioral therapy is frequently used in these situations. In certain situations, your child’s therapist may recommend the addition of safe and helpful medication.

            School should be a safe space for children, but after recent events, apprehensiveness in regard to attending school is understandable. Symptoms of Anxiety

Elementary school students may present with different symptoms of anxiety each day before, during, and after school. Physical manifestations of anxiety may include stomachaches, restlessness, heart palpitations, and complaints of not feeling well enough to attend school. These children often have difficulty falling and staying asleep and may refuse to attend school in the morning. While in the classroom, these students may show difficulty concentrating, show excessive preoccupation with performance, or may perform poorly as a result of excess worry.

Types of Anxiety Presented

Separation anxiety is characterized by excessive worry about being separated from caregivers and commonly affects students of young ages. Social anxiety is another disorder that can be found in children, impacting their ability to participate in the classroom and socialize with their classmates. Another disorder is generalized anxiety disorder (GAD) which affects students who worry about a wide variety of school issues. For instance, students with GAD may struggle with academic perfectionism. Although typically harder to identify in a school setting at a young age, young students may present with symptoms of obsessive-compulsive disorder. Students with this disorder may perform compulsive rituals or behaviors to ease their anxiety. Other anxiety disorders that may affect a student are selective mutism and specific phobias. These often impact academic and social performance.

Treatment Options

Psychotherapy can help children struggling with anxiety. Cognitive behavioral therapy (CBT) is the most commonly used treatment option. This therapy focuses on negative patterns of thoughts and addresses how thoughts affect the way the child feels. Parents of children with anxiety disorders can benefit from speaking to a child psychologist about how they can help. Medications may also be used to ease symptoms for a wide-variety of anxiety disorders. Why Men are less likely to Seek Mental Health Services

Recent research studies have found that men are less likely to seek mental health assistance compared to their female counterparts. Men are also less likely to disclose a mental health problem to friends and family. This isn’t because men are immune to mental illnesses but rather something larger is deterring men from reaching out for help. So what can psychologists attribute this to? The answer is toxic masculinity.

Toxic masculinity refers to harmful social norms about how men should behave that lead to misogyny, homophobia, violence, and mental health issues. In our society, we typically categorize women as the emotional figures and men as the strong and brave figures. Due to these social norms, people assume that men do not need mental health services as it is perceived as unnatural for men to discuss their emotions and feelings. Men themselves see seeking help as a sign of weakness, tarnishing their masculinity.

The media contributes to this societal problem as well, as a majority of mental health advertisements are targeted towards women and feature women. This further perpetuates the idea that mental health is a women’s issue, rather than a human issue. It is time we stop gendering mental health and remove the stigma surrounding men’s mental health.

Why it Matters

In 2020, men died by suicide 3.88x more than women. White males accounted for 69.68% of suicide deaths in 2020. It is abundantly clear that there are men struggling with mental health conditions, so much so, that they believe suicide is the only answer. If we continue to feed into toxic masculinity, men will continue to avoid help when they need it most. So what can we do about it?

Ways to Help

  1. Educate yourself and others – mental health problems are medical problems that can impact anybody regardless of gender.
  2. Talk openly about mental health – sharing personal experiences with mental health problems can make others more comfortable talking about their own experiences.
  3. Show compassion for those with mental health problems – showing compassion for those suffering can help reduce feelings of shame.  
  4. Check on the men in your life – Men are less likely to share their feelings compared to women, so work on having honest conversations with the men in your life. Let them know that you are there for them and stay alert of any concerning changes in behavior.

Depression and Anxiety Post Retirement

While many people look forward to retirement and its freedom, it can also trigger anxiety, stress, and depression. In fact, the chance of a person facing clinical depression increases by 40% after retirement. People tend to give lots of thought when planning their retirement; whether it’s traveling the world, pursuing new hobbies, or spending more time with family and friends. However, they often overlook the psychological impact of retiring from work. While many new retirees find retiring to be a great relief from escaping the daily grind, they also find that after several months they may miss the sense of identity, meaning, and purpose that came with their job. They lose the structure that their job gave their days, as well as the social-aspect of having co-workers. Retirees may now feel bored and isolated, rather than free, relaxed and fulfilled. They may even grieve the loss of their old life, and feel stressed or worried about how they will now spend the future.

Retiring is a major life change, and it can seriously take a toll on one’s mental health. Seeking help from a professional can help provide coping mechanisms with these challenges of retirement. There are certainly healthy ways to adjust to this new chapter in your life, and it should be an exciting time, not a negative experience. Agoraphobia is an anxiety disorder that causes an individual to feel an intense fear of being overwhelmed, unable to escape, or unable to get help. Due to this intense fear and anxiety, people will often avoid new places and unfamiliar situations. New places or unfamiliar situations include: open or enclosed spaces, places outside their home, crowds, and public transportation. Usually, Agoraphobia begins with a stressful event that makes an individual feel distressed and in turn, limits their contact with the world. This limitation of contact causes avoidant-behaviors with time the individual remains confined to their home. Agoraphobia is also caused by a stressful life event that triggers a panic attack. Due to the unpleasantness of panic attacks, the individuals will avoid any place or situation that will trigger another panic attack. These instances show that agoraphobia develops over time, rather than happening all at once.

The signs of agoraphobia are similar to a panic attack disorder, but the following symptoms can still occur:

  • Chest pain or rapid heart rate
  • Upset stomach
  • Dizziness or lightheadedness
  • Sudden chills or flushing
  • Fearfulness
  • Hyperventilation
  • Excessive sweating

Psychotherapy: Benefits of Rational Emotive Behavior Therapy (REBT)

Rational emotive behavior therapy (REBT) is a type of cognitive behavioral therapy (CBT) that focuses on helping people recognize and change negative thinking patterns into positive, healthier ones. This method is an action-oriented approach helping to overcome any psychological problems or mental distress. The goal of REBT is to help people respond rationally to situations that would cause stress, depression, or other negative feelings. How does it work?

The ABC model is one concept of REBT. The model suggests that we may blame external situations for our unhappiness and it is our interpretation of these situations that truly causes the psychological distress. ABC stands for:

  • A: Activating event, when something happens in your environment
  • B: Belief, describes your thoughts about the situation
  • C: Consequence, which is your emotional response to your belief

With REBT, your therapist will help you learn how to apply the ABC model in your daily life. Your therapist may help you identify the activating event before encouraging you to figure out which belief led you to your negative feelings. Once you’ve identify the underlying issue, your therapist will work with you to change those beliefs and your emotional response towards the issue. Before changing your belief, a process called disputation takes place where your therapist will challenge your irrational beliefs using direct methods such as asking questions which causes you to re-think or have you imagine another point of view that you may have not considered before. REBT can help with Anxiety, Social anxiety disorder, distress, Depression, Disruptive behavior in children, Obsessive-compulsive disorder (OCD) and Psychotic symptoms.

  • Improves health and quality of life

REBT helps you understand that you are worthy of self-acceptance no matter what even if you or others are struggling; there is no need for shame or guilt because everyone makes mistakes and it’s normal to feel some discomfort. REBT gives insight that others are also worthy of acceptance even if their behavior involves something you don’t like. Overall, REBT helps you have a healthy emotional response on learning from a situation and moving on. This allows you to understand that negative things will sometimes happen in life therefore there is no rational reason to always expect it to be positive when faced with a situation.

Generalized Anxiety Disorder (GAD) is a common anxiety disorder in the United States, currently impacting 3.1% of the population; as many as 5.7% of U.S. adults report experiencing this disorder at some point in their lives. GAD is characterized by feelings of excessive worry which have no particular trigger. This anxiety can be felt about school, work, social interactions, or even common, everyday events. These behavior patterns and cognitive issues become disordered when they begin to disrupt normal functioning.

Symptoms of GAD include the following: restlessness, being wound-up or on-edge, fatigue, trouble concentrating, irritability, muscle tension, and sleeping problems including difficulty falling/staying asleep or unsatisfying sleep. At least three of these symptoms must be present for at least six months for a patient to be diagnosed with GAD.

Treatment:

Cognitive Behavior Therapy (CBT) is a type of therapy that is particularly helpful in treating anxiety disorders, including GAD. This therapy involves teaching patients different ways to approach anxiety inducing situations by changing how they think, behave, and react to them. It also helps to implement social skills in patients.

Medications are another form of treatment for GAD. It should be noted, however, that medications are used to help decrease symptoms of a disorder and do not cure it entirely. Anti-anxiety and anti-depressant medications are commonly used to treat GAD. Anti-depressants work for both depression and anxiety by altering chemicals in the brain, specifically serotonin and norepinephrine. They help to regulate mood and relieve symptoms associated with these disorders. Selective serotonin reuptake inhibitors (SSRIs) and serotonin norepinephrine reuptake inhibitors (SNRIs) are the most commonly used anti-depressants. Anti-anxiety medications, such as benzodiazepines, are effective in treating anxiety symptoms quickly. They are often used as a second option when anti-depressants are not enough to relieve symptoms.

Eating disorders are characterized as obsessive, repetitive thoughts, and ritualistic behaviors. Obsessive-compulsive disorder (OCD) is a common, chronic long lasting disorder that characterizes uncontrollable thoughts or behaviors that an individual may feel they need to repeat certain things over and over. Statistics show that people with eating disorders are more likely to show signs of OCD, due to the overlapping traits of both OCD and eating disorders.

Understanding the similarities and differences between eating disorders and OCD can help develop a more comprehensive understanding of a patient that presents both of these disorders. People with an eating disorder may experience intrusive thoughts about food and body image, and may develop ritualistic behaviors. Examples of ritualistic behavior pertaining to eating disorders include body checking for any changes in shape or size, frequent weight checking, and skipping meals. An important distinction between OCD and eating disorders lies in the relationship between the thought and action of the individual.  People with OCD are typically interested in ridding themselves of their thoughts and feelings whereas; people with eating disorders may feel more tied to the components of this disorder and feels as if it is a part of their identity.

Since both eating disorders and OCD share overlapping diagnostic characteristics, treatments will look similar. Both exposure therapy and cognitive behavior therapy are very helpful treatments for both eating disorders and OCD. Exposure therapy is a psychological treatment that involves exposing the patient to the anxiety source or its context without the intention to cause any danger. Also, cognitive-behavior therapy is a treatment approach that helps you recognize negative or unhelpful thought patters.

 As universities are opening up and students are returning back to the classroom, students may feel increases in test anxiety as they return to an academic setting. While some anxiety may be a good motivator for studying, a crippling amount of anxiety can result in a decrease in grades and an increased risk of mental health issues. Here are some tips to conquer testing anxiety.

  • Study Smarter, Not Harder: Make sure you’re prepared for the test. Don’t cram and don’t spend too long stressing over the subject. Ask friends and family for help, and set goals to help you reach your potential in different subjects.
  • Focus on the positives: Negativity can be a self-fulfilling prophecy. If you tell yourself you’ll do poorly, you’ll end up not having the motivation to study and thus do poorly. Practice positive self-talk and set realistic goals for yourself.
  • Build Good Habits: Manage your time wisely. Make sure to get enough sleep the day before the test and eat something nutritious the morning of the test.
  • Do Relaxation Exercises: There are a number of ways to alleviate physical symptoms. Do breathing exercises, count backwards from one hundred, and meditate. Find out which relaxation technique works best for you.

            As everyone returns to the classroom there will be an adjustment period. Be proactive in helping your test anxiety and practice the methods that work best for you.

Post Traumatic Stress Disorder: Understanding the Diagnostic Criteria

Post- traumatic stress disorder is a psychiatric disorder that individuals can develop after experiencing a traumatic event. Traumatic events are defined by incidents that have the capacity to provoke fear, helplessness, or horror in response to the threat of injury or death. Further, as stated in the Diagnostic and Statistical Manual of Mental Disorders (5th ed; DSM- 5; American Psychiatric Association, 2013), one must display 4 types of symptoms to be diagnosed with post- traumatic stress disorder.

The first type is symptoms of re-experiencing the event in which the individual has unwanted recollections of the event (i.e. nightmares). The second type is symptoms of avoidance that include avoiding thoughts and feelings in regard to the incident as well as external reminders of the event (i.e. people, places). The next type of symptoms is symptoms of negative alterations in cognition and mood associated with the traumatic event (i.e. exaggerated negative beliefs/ expectations about oneself). Lastly, individuals will experience marked alterations in arousal and reactivity associated with the event (i.e. exaggerated startle response). Additional criteria includes length of symptoms in which symptoms should persist for longer than one month, severity of symptoms in which they should cause significant clinical distress, and cause of disorder such that symptoms should not be linked to the effects of drugs or substances.

It is also important to note that individuals can have a “delayed expression” of post-traumatic stress disorder (PTSD), which, according to the DSM-5, occurs when the “full diagnostic criteria [of PTSD] are not met until at least 6 months after the event” (APA, 2013). To restate, symptoms can manifest long after this 6 month window. The DSM-5 also states that having had a prior psychiatric disorder such as depression, and emotional problems such as anxiety problems, serve as risk factors in developing PTSD after one experiences a traumatic event. This relationship also works in reverse; experiencing a traumatic puts one at an increased risk for developing other psychiatric disorders such as generalized anxiety disorder (GAD), and at risk for the worsening of GAD symptoms if they had been diagnosed prior to the PTSD diagnosis.
