Sleep Disorder: Narcolepsy

Narcolepsy is a chronic sleep disorder that affects daytime activities. It is characterized by overwhelming drowsiness and sudden attacks of sleep. Narcolepsy affects both men and women equally in roughly 1 in 2,000 people and can be passed down genetically, but the risk of a parent passing this disorder to a child is very low. Symptoms usually start to develop between the ages of 10- 30 years old and worsen for the first few years. The symptoms of narcolepsy will remain constant throughout life.

Some symptoms of narcolepsy include:

  • Excessive sleepiness
  • Sleep paralysis
  • Hallucinations
  • Episodes of cataplexy (partial or total loss of muscle control that is often triggered by strong emotions such as laughter and joy)

Other symptoms include:

  • Transition to REM sleep is quick, usually 15 minutes
  • Insomnia
  • Restless leg syndrome
  • Sleep apnea
  • Automatic behavior (falling asleep while doing an automatic task, like driving, and continue performing task after falling asleep. When waking up and not remembering what they did)

Narcolepsy is a sleep disorder that seriously disrupts everyday life. Most common being:

  • Stigma of the condition- others might view individuals with this disorder as lazy or lethargic
  • Physical harm- increased risk of being in a car accident if a sleep attack occurs when driving
  • Low metabolism- individuals may be more likely to be overweight

Unfortunately, the exact cause is still unknown and there is no cure for narcolepsy. However, medications (stimulants), lifestyle changes, and support from others can help manage symptoms.

Seasonal Depression, clinically known as Seasonal Affective Disorder or SAD, affects about five percent of American adults annually. SAD is a type of depression where the weather and seasonal changes causes one to feel depressed. SAD symptoms most commonly flare up around the late fall to the early spring months. During this time of the year, the sun is out shorter and its rays are less intense. Psychologists hypothesize that the lack of sunlight contributes to SAD by affecting healthy hormonal balances. Although most cases of SAD occur during the late fall to early spring seasons, some people do have seasonal depression during the warmer spring and summer months. Studies show that alcohol consumption and depression go hand-in-hand, which can be particularly harmful when suffering from SAD.

Symptoms of SAD:

  • Feeling of sadness or depressed mood
  • Lack of motivation
  • Marked loss of interest or pleasure in activities once enjoyed
  • Changes in appetite; usually eating more, craving carbohydrates
  • Change in sleep; usually sleeping too much
  • Loss of energy or increased fatigue despite increased sleep hours
  • Increase in restless activity (e.g., hand-wringing or pacing) or slowed movements and speech
  • Feeling worthless or guilty
  • Trouble concentrating or making decisions
  • Thoughts of death or suicide or attempts at suicide

Fortunately, there are many different treatments to help those with SAD reduce their symptoms. Psychotherapy and medication are helpful for those suffering from SAD. Cognitive Behavioral Therapy or CBT helps patients change unhealthy thought patterns and behaviors that may contribute to their symptoms. Psychiatrists most often prescribe antidepressants such as Lexapro or Prozac to help those with SAD. A new type of therapy, light therapy, has proven to help those with SAD by exposing them to artificial light which takes the place of the light they are missing on short fall or winter days. One usually sits in front of the light box for about twenty minutes each morning. Patients usually start feeling better after two to three weeks of light therapy. In addition to the services professionals can provide to help, there are lifestyle choices that one can make to lessen the effects of SAD such as avoiding drugs and alcohol, getting regular exercise, getting a healthy amount of sleep and eating a healthy diet.

Burnout

                Recently, the World Health Organization (WHO) has identified “burnout” as a syndrome and has added it to the WHO’s International Classification of Diseases. This legitimization of burnout as a syndrome provides credibility to a person who is over working themselves emotionally, physically, and intellectually.

According to Dr. Suzanne Degges-White’s article on Psychology Today, the symptoms of burnout include:

“1.Feelings of energy depletion or exhaustion

2.Increased mental distance from your job or feelings of negativity or cynicism related to your job

3.Reduced professional efficacy—or, in laymen’s terms, doing a poor job on-the-job”

Dr. Degges-White explains that although most symptoms of burnout may resemble other disorders associated with depression and anxiety, burnout is focused on direct feelings towards your job.

Mindfulness practices may decrease feelings of burnout. Activities such as regular exercise, yoga, and mindfulness meditation may help to stabilize and encourage balance in your life. It is also important to try to sleep and rest as much as you can. Sleeping is a restorative process and helps to promote a healthy mind and body. Because burnout is due to chronic workplace stress, it will not go away by taking a vacation or escaping the workplace for a few days. Burnout is something to be taken seriously and if it is impairing your lifestyle, seek treatment from a psychiatric professional.

              ADHD or Attention Deficit Hyperactivity Disorder is a mental condition that is characterized by inattentive, hyperactive, and impulsive behavior. ADHD is usually diagnosed in elementary school, but symptoms can appear as early as three years old.  About four to five percent of children have ADHD and about 60 percent of these children carry their symptoms into adulthood. In order to be diagnosed with ADHD, one must have had symptoms present before the age of 12. People with ADHD oftentimes have trouble paying attention or maintaining a regular level of activity, which can cause them to be disorganized or experience difficulty at work, school, and in relationships. Those with ADHD can sometimes feel anxious or depressed if their symptoms cause dysfunction in their life.

Symptoms of Inattention:

  • Making careless mistakes in schoolwork, at work, or during other activities
  • Having problems sustaining attention during conversations, lectures, reading, etc.
  • Not seeming to listen when spoken to directly
  • Having problems organizing tasks and activities; poor time management and oftentimes messy work/living spaces
  • Frequent loss of necessary items such as keys, school supplies, wallets etc.
  • Forgetfulness

Symptoms of Hyperactivity-Impulsivity:

  • Frequent fidgeting and squirming
  • Leaving seats in situation when staying seated is expected
  • Restlessness
  • Trouble engaging in hobbies quietly
  • Rapid nonstop speech
  • Speaking without waiting for a turn in the conversation
  • Having trouble waiting for one’s turn
  • Interrupts others

Symptoms of ADHD are oftentimes greatly improved use of medication.  Medications prescribed by psychiatrists and psychiatric nurse practitioners such as stimulants, help those with ADHD focus their thoughts and reduce hyperactivity. In addition to medication, psychotherapy is oftentimes helpful for those with ADHD cope with every day struggles presented by the disorder.

Caregiving is the responsibility to provide unpaid care for an adult or child which may involve shopping, housekeeping, toilet assistance, bathing, coordinating medical treatments, and managing a person’s finances. One of the main struggles a caregiver must learn to cope with is that caregiving is a 24/7 job, in addition to any other professional or personal endeavors that person is pursuing. While the patient is most commonly a loved one and family member, the caregiver must remember to take care of themselves. Additionally, a caregivers’ own health and daily needs are oftentimes neglected and this can be detrimental to that individual’s well-being. “Caregiver stress” is when caregiving becomes too overwhelming.

The most common feelings associated with caregiver stress include:

  • Exhaustion
  • Frustration
  • Loneliness
  • Guilt
  • Anger
  • Burden, feeling of weight on your shoulders
  • Anxiety

It is important to remind a caregiver to set aside time for themselves, to focus on their own mental and physical health, as well as other pressing needs. Meditation, reading, and yoga are a few ways in which a caregiver can relax. It would also be helpful to seek other forms of aid for that patient, whether that be the help of other family members, daily visits from nurses, or considering putting your loved one into a nursing or residential assisted living home so they can be cared for 24/7 by professionals. Our culture is obsessed with weight, thinness, exercise, and beauty. Studies show that women under the age of 19 are particularly vulnerable to the problematic effects of social media; about 60% of girls have a desire to lose weight even though they are within the healthy weight range ( Morris & Katzman, 2003). As a result, many people have developed unhealthy ways of eating and exercising for dealing with the pressure to look a certain way. These habits can seem minor at first, but can quickly develop into an eating disorder. While disordered eating habits do not meet the clinical criteria for an eating disorder, they are still unhealthy and potentially damaging.

Signs of Disordered Eating Habits:

  • Self-worth or self-esteem based highly or even exclusively on body shape and weight
  • A disturbance in the way one experiences their body i.e. a person who falls in a healthy weight range, but continues to feel that they are overweight
  • Excessive or rigid exercise routine
  • Obsessive calorie counting
  • Anxiety about certain foods or food groups
  • A rigid approach to eating, such as only eating certain foods, inflexible meal times, refusal to eat in restaurants or outside of one’s own home

It is important to recognize the signs of disordered eating and try to eradicate them before they become potentially harmful. Clinicians advise to quit fad diets because they are extremely restrictive and often result in binge eating. Obsessive exercise focused on “fat-burning” or “calorie-burning” should also be avoided and replaced with physical activity that is more focused on enjoyment. Another tip psychologists give is to avoid weighing yourself every single day. Weight can fluctuate about 2-5 lbs. a day, so fixating on a specific number in order to be healthy isn’t helpful. If disordered eating habits suddenly get worse, or start to impact one’s daily functioning, one should seek help.

Psychosomatic disorder affects both mind and body and it occurs when a physical disease is exacerbated or caused by mental factors or vice versa. Psychological factors can cause physical symptoms when there is no physical disease. There are many factors that are involved with psychosomatic disorders, such as

  • Biological traits
  • Genetic and environmental factors
  • Family influences
  • Learned behavior

Some symptoms of psychosomatic disorders include:

  • Generalized pain
  • Weakness
  • Fatigue
  • Shortness of breath

There are many physical diseases and conditions that are prone to be exacerbated by psychological factors. Some conditions include:

  • Hypertension
  • Constipation
  • Diarrhea
  • Dyspnea
  • Tachypnea
  • Tachycardia

Cognitive behavioral therapy is often the treatment for psychosomatic disorders and it helps individuals cope with their problems (eg. stress, anxiety, depression) and understand that their mental health is connected to their physical health. Most healthcare professionals try to treat the individual, taking into account the mental, social, and physical factors that may contribute to a disease.

Bulimia Nervosa is an eating disorder known for episodes of out of control eating and subsequent self-induced methods of purging in order to prevent weight gain. Binging is considered an irresistible compulsion and on average happens about once a week, but can also occur several times a week. Bulimia is most common in women and typically begins in their teens to early 20s. The cultural obsession with a perfect body image, fueled by social media models with unattainable body “goals”, is harmful to young people’s self-esteem and body image. Social media perpetuates a cultural and social expectation of women needing to be thin in order to be beautiful, and this negatively impacts our younger generations.

Signs of Bulimia include:

  • Yellow teeth, erosion of dental enamel
  • Gastrointestinal problems
  • Unexpected weight gain
  • Hair loss
  • Lack of bowel movements
  • Lack of energy
  • Dehydration
  • Red, blistered, or scabbed knuckles

Some typical purging methods include:

  • Vomiting
  • Taking laxatives or diuretics
  • Excessive exercise

Some typical restrictive eating symptoms include:

  • Fasting
  • Disordered Eating

Research has shown that people with Bulimia Nervosa have comorbid mental illnesses such as anxiety, depression, or substance use disorder. Bulimia is also related to personality disorders which effect one’s social, personal, behavioral, and emotional interactions. Bulimia is best treated through a primary care physician, a nutritionist, and a mental health clinician. Cognitive behavioral therapy as well as antidepressants are extremely helpful for those recovering from Bulimia. It is important that schools address eating disorders at an early age in order to educate and bring awareness to young people.

                It’s about time that college students may return back to their parent’s home for the summer. At first, many parents are excited for their children to go to college or begin their professional career, however, it is common for parents to feel sad, lonely, and experience grief when their children leave their childhood home. Typically women will struggle more than men during these times, especially when women were full-time mothers. It is important to know that Empty Nest Syndrome is a transitional period in life rather than a clinical disorder or diagnosis. Although the change brings about conflicting emotions, parents are encouraged to support their children in the gradual process towards adulthood.

Symptoms of Empty Nest Syndrome:

  • Sadness
  • Excessive crying
  • Loss
  • Depression
  • Loneliness
  • Distress
  • A loss of purpose and meaning in life
