ADHD: Gender Disparities in ADHD Diagnoses

Attention deficit hyperactivity disorder (ADHD) is characterized by hyperactivity, impulsive behaviors, difficulty paying attention, and high distractibility. ADHD is one of the most common disorders of childhood, but boys are much more readily diagnosed than girls. It is believed that the prevalence of ADHD across genders is approximately equal, even though formal diagnoses strongly favor boys. Females are often underdiagnosed with ADHD and misdiagnosed with other disorders, such as bipolar, depression, and anxiety. The symptom profile for ADHD is the same across genders, but the behavioral presentation of symptoms tends to differ greatly.

Females are more likely to exhibit more internalized behavioral symptoms, such as:

  • Daydreaming
  • Losing focus
  • Forgetfulness
  • Anxiety
  • Insomnia
  • Impatience
  • Low self-esteem
  • People-pleasing
  • Deep emotional reactions/overwhelm

Males, on the other hand, often display more external behaviors, such as:

  • Hyperactivity
  • Disruptive behavior
  • Interrupting people during conversations
  • Losing items often
  • Anger outbursts

The external manifestation of ADHD symptoms makes the disorder more likely to be noticed in young boys by parents, teachers, and peers. Since young girls usually do not show such external symptoms, they tend to not receive a formal ADHD diagnosis until much later in life (if at all).

Generally thought to be a disorder specific to school-aged boys, attention deficit disorder (ADHD) has shown to be relatively prevalent in girls as well. The disorder includes 3 subtypes: hyperactive and impulsive  (HI), inattentive, and combination. ADHD is commonly associated with the HI subtype, which is most commonly exhibited by boys. Contrastingly, girls tend to exhibit the inattentive subtype. By nature, inattentive features are not as overtly obstructive as that of hyperactivity and impulsivity, often causing them to go unnoticed. In fact, studies estimate that 75% of girls with attention deficit disorder never get diagnosed. Additionally, it is theorized that societal norms, such as gender roles, might also be a factor in this disparity; since many overt characteristics of ADHD do not align with female gender norms, such as the tendency to be disorganized or interrupt others speaking, girls with the disorder tend to suppress the tell tale signs. 

The result of undiagnosed attention deficit disorder can be detrimental, as it can lead to mental health consequences in adulthood. This is largely due to the fact that girls tend to internalize mistakes. This internalization leads to negative internal dialogues, which puts girls with ADHD at higher risk for eating disorders, anxiety disorders, and depression. It is imperative to be aware of the ways in which the disorder manifests differently in girls. For example,

A girl with ADHD might:

-be more more easily irritated, or sensitive to certain sounds/feelings

-talk significantly more than her peers and often interrupt others

-struggle to commit to completing tasks or activities

-often make “careless” errors

-seem to be especially disorganized

-tend to be forgetful

Laziness: The Harmful Effects of the Term “Lazy” on Mental Health

               “Lazy” is a common uncomplimentary term in modern vocabulary for when someone is unproductive. Think back to a time a group member failed to pull their weight in a group project, or a time someone procrastinated severely, leaving everything for the last minute and creating a poor final product. Consider even a time where you witnessed someone who, by early afternoon, was seemingly unable to bring themselves to get out of bed to start the day.

Whether it was yourself or someone else that you imagined, it’s easy to write off everyone in those examples as lazy. However, there’s a major issue with doing that – “laziness” is often not the cause of these situations. Rather, many mental health conditions can create issues that simulate behaviors identical to laziness.

Take, for example, disorders such as attention deficit hyperactivity disorder (ADHD), depression and other mood disorders, insomnia and other sleep disorders, and anxiety disorders including obsessive compulsive disorder (OCD) and generalized anxiety disorder (GAD). Each of these conditions can negatively impact a person’s ability to be productive, making them appear lazy. People with ADHD can often find themselves imagining all of the tasks they could be engaging in at once and becoming so overwhelmed they feel almost paralyzed. People with depression and other mood disorders often lack the mental energy to accomplish anything. Similarly, people with insomnia and other sleep disorders often lack the physical energy to accomplish anything. People with GAD may have a crippling fear that they won’t be good enough at something, preventing them from attempting to do the task in question. People with OCD may have a crippling (rational or irrational) fear that something bad will happen if they do specific things, preventing them from doing those things.

               All of these explanations are generalized and therefore may not apply to everyone with each listed disorder, or be the only applicable factor for each disorder’s effect on laziness. However, if you or someone you know has been consistently labeled as lazy, remember that “laziness” is often more than how it appears on the surface, and that actively struggling with mental health does not make a person a failure.

Social Media and Attention Span

People have spent increasingly more time on social media throughout the years which has led to shorter and shorter attention spans. This is because of click bait material and multitasking.

Social media is designed to grab people’s attention and get people to their next click so that people stay online for longer. Instead of publishing detailed, meaningful articles, people are now publishing more sensational, controversial pieces to get people to click. Because the material is very loud and polarizing, people have an urge to switch to new material, so new information is constantly competing for attention, reducing our attention span. This can be addictive in nature and teaches people to focus on engaging material for a short period of time and stay on social media for a long period of time. 

Additionally, social media is something that is commonly used while completing another task. When a person is multitasking, attention span is reduced. The effects are especially detrimental for younger people who are more susceptible to developing bad habits. The average attention span in 2000 was 12 seconds and is now 8 seconds, which is 1 second shorter than the attention span of a goldfish! This is because it takes greater cognitive effort to switch between tasks than it does to maintain the same level of concentration on one task. Research has also shown that episodic memory can be significantly reduced when multitasking. 

Some ways to prevent declining attention spans are:

  • Implementing a “no phone at dinner” rule
  • Complete one task at a time 
  • Put your phone away while working

Anxiety, Depression, Eating Disorders, ADHD, Et al: How to Support a Friend with Mental Illness

When helping a friend with a mental illness, the first step should be assessment of their symptoms. Sometimes they just might be going through a difficult time, but if certain common symptoms associated with mental health issues persist it is imperative to respond sensitively. Majority of the time, friends will just want to know they have your support and that you care about them. A good way to show your support is by talking to them. If you provide a non-judgmental space for them to speak about their issues it will help encourage them to be open with their problems. Let them lead the conversation and don’t pressure them to reveal information. It can be incredibly difficult and painful to speak about these issues and they might not be ready to share everything. If you aren’t their therapist do not diagnose them or make assumptions about how they are feeling, just listen and show you understand. If someone doesn’t want to speak with you, don’t take it personally, just continue to show them you care about their wellbeing and want to help as much as possible. Just knowing they have support can give them the strength they need to contact someone who can help them.

If a friend is having a crisis, such as a panic attack or suicidal thoughts, you must stay calm. Try not to overwhelm them by asking a lot of questions and confronting them in a public setting. Ask them gently what would be helpful to them right now or reassure them. If they hurt themselves, get first aid as soon as possible. If someone is suicidal, contact the suicide hotline at 800-237-8255 immediately.

The best way to help someone is by connecting them to professional help. By expressing your concern and support you can show them that they can get help and their mental health problems can be treated.

ADHD: Why is it underdiagnosed in women?

Attention-deficit hyperactivity disorder (ADHD) is a neurodevelopmental disorder characterized by persistent issues with concentration and attention, as well as frequent hyperactive and impulsive behavior. For many years, ADHD was believed to only affect young boys, but it is now known to affect both men and women of all age groups. Even today, though, ADHD is often underdiagnosed in girls, many of whom only get a diagnosis much later in life or not at all.

This underdiagnosis of ADHD in girls has multiple causes. Primarily, ADHD presents differently in girls than boys, with boys demonstrating symptoms more stereotypically associated with the disorder. When most people imagine ADHD, they think of a young child, calling out randomly in class, bouncing up and down in their desk, fidgeting, and unable to remain still. This image can make it difficult to pick up on ADHD in girls, who display it differently.

Girls tend to display more internal symptoms than external symptoms, as boys do. This is mainly through inattentiveness and daydreaming. Internal symptoms are more subtle and, therefore, difficult to pick up on in order to make a proper diagnosis. Additionally, girls with ADHD often display intense emotional responses, rather than typical impulsivity and hyperactivity. This can be misinterpreted as immaturity, so it is not often picked up on as a symptom of ADHD.

Oftentimes, as well, girls have co-occurring conditions that can cause ADHD to be overlooked. Girls and women with ADHD are more likely than their male counterparts to suffer from depression and anxiety. Both of these conditions can present with some symptoms similar to ADHD, such as restlessness and issues with concentration. All these factors combined make it difficult for women and girls to get the proper diagnosis and treatment for their ADHD, but with awareness and education, this can change.

ADHD: Treatment

Attention-deficit hyperactivity disorder, also known as ADHD, is characterized by a pattern of inattention, impulsivity, and hyperactivity that impedes normal functioning and activity. For example, someone who has ADHD may be unable to focus on a task or will constantly move around and fidget.

Diagnosis of ADHD can only be given by a licensed clinician like a psychologist or psychiatrist. Typically, ADHD will only be diagnosed if there is a repeated pattern over a period time of inattention and hyperactivity. In children, a diagnosis requires a child falling behind in development. Adults who suffer from ADHD may have poor academic performance in school or issues at work as a result of inattention.

Risk factors for ADHD include genetics, exposure to environmental toxins during pregnancy, behavioral toxins, and low birth weight. Treatment for ADHD may include therapy or medication to reduce symptoms and improve functioning. When medication is needed, doctors typically prescribe a stimulant that increases dopamine and norepinephrine levels in the brain. This improves thinking and attention.

Therapy can help a person suffering from ADHD develop coping mechanisms like mindfulness or possibly meditation. Simple strategies like making lists, keeping a stable routine, utilizing a calendar, and creating reminder notes may help a person with ADHD improve functioning.

Attention Deficit Hyperactivity Disorder

The most commonly diagnosed disorder among children and young adolescents is ADHD, which stands for Attention Deficit Hyperactivity Disorder. Among adults, only 4% are diagnosed with this order. The most significant common symptoms for ADHD are: hyperactivity, inattention and impulsivity. Each symptom has their own signs that professionals look out for. For example:

Hyperactivity:

  • Talks excessively
  • Always on the go
  • Restlessness

Inattention:

  • Misses details
  • Easily distracted
  • Often forgetful

Impulsivity:

  • Tough time waiting
  • Interrupts others
  • Blurts out answers

Along with the symptoms listed above, problems with organization, time management, and controlling emotions are also pat of ADHD. Procrastination, isolation and difficulty concentrating are challenges people with ADHD may also face. It is also likely for them to have trouble with self-esteem, relationships and addiction. ADHD can cause a person to have problems at school and work as well.

ADHD may be caused by genetics, brain injuries, chemical imbalance and other causes. Poor prenatal development such as smoking while pregnant, can be a factor behind ADHD too. If diagnosed with ADHD as a child, it will continue on to adulthood. Over the years, the disorder does not go away, a person just learns to manage their ADHD.

ADHD in Teens: Why Adult Support is Essential

Common symptoms of ADHD lead people to believe that students that have been diagnosed willingly choose to not complete assignments or be inattentive during class. However, symptoms like forgetfulness, procrastination, distractibility, poor time management, and having a difficult time writing are exactly what prohibits a student from achieving academic success. By definition, these teens cannot manage their time on their own, not even with all the motivation in the world. And although these students want to be treated normally, it is important to intervene in order to teach them how to manage their ADHD and their lives.

Wanting to be “normal” often prevents these students from seeking the help that they need with assignments and time management. While school counselors mean well with their open office doors, it’s important to recognize when a student with ADHD is falling behind. The student, even if a need for help is stated, will often not follow through with appointments due to the lack of executive control and interfering symptoms like distractibility and impulsivity. Setting up a comprehensive academic plan with the teen takes the efforts of parents and teachers who constantly check in to make sure he or she is on track. How you can support a student with ADHD:

  • Promote independence: the end goal is to have the teen be able to keep track of assignments, organize himself, be able to take notes, study well, and mange his time by being able to break down projects. Have the student come up with his own plan for the school year and check in if it seems off base. Once a well-oiled routine is put in place, check in regularly.
  • Intervene early: always prompt students to solve academic problems immediately. It’s better to catch small mistakes and setbacks early on than to wait until they snowball out of control.
  • Provide guidance: you should aim to collaborate with the teen in finding solutions to any problem that arises, however, most need direct instruction at first. They need to be shown exactly what the outcome needs to be before they can decide on the best way to arrive at the desired solution.
  • Take the lead: students should be under adult supervision at all times. At school, it’d be teachers and counselors and at home it’s their parents or guardians.
  • Gradually withdraw supports: adult supervision shouldn’t be completely withdrawn until the student proves capable. Symptoms can cause poor executive function well through college because it largely interferes with academic skills no matter their age.

ADHD: How Medication Helps Your Children

Stimulant drugs, such as Adderall and Ritalin, have been used for numerous years as the frontline treatment of attention-deficit/hyperactivity disorder (ADHD) in children. ADHD is a disorder characterized by the following symptoms: inattentiveness, distractibility, hyperactivity, and impulsivity. The five to seven percent of children diagnosed with ADHD have difficulty concentrating, become disruptive and defiant, and have a hard time getting along with family members, peers, and teachers. These behaviors are clinically proven to improve with the use of stimulant drugs.

Treatment with stimulant drugs can improve classroom performance and behavior in children diagnosed with ADHD. But how does the medication do this? The medication works by targeting and improving specific cognitive processes, such as working memory, which is your ability to hold and manipulate information in your mind. Stimulants help kids store more information in their working memory, while also helping them to inhibit negative behaviors (i.e., raising their hand when they know the answer instead of shouting it out). Improvement of these cognitive processes also helps to increase attention span, which allows kids to stay on task for longer periods of time. If you believe medication is not the best option for your child, behavioral treatment and parent training can also be beneficial in improving these cognitive processes. Children can be trained to improve their self-control and sustain concentration.
