Borderline Personality Disorder (also known as BPD) is a personality disorder that is categorized in the DSM-V by intense fears of abandonment, emotional instability in everyday life and relationships, and a distorted sense of self. Studies have found that while BPD is equally as prevalent in men as it is in woman, statistically speaking, seventy-five percent of people with BPD are female. So why are females most likely to get diagnosed with BPD?

While the criteria for men and women with BPD are the same, men and women are more likely to exhibit different maladaptive behaviors. Women will most likely self-harm through eating disorders and cutting while men will more likely self-harm using substances and head banging. Due to this, men are more likely to end up in prison for violent behavior while women are more likely to seek mental health resources to deal with their self-harm.

There’s also a societal stigma around men receiving mental health help. While women are seen as emotional creatures, men are seen as unemotional and are told to “suck it up” or “man up” when they’re upset. Another factor is that the maladaptive behaviors of BPD are discouraged in women and encouraged in men. Men who have tumultuous relationships, aggressive behavior, and exert violent behavior are more likely praised in society than women who express the same symptoms.
Despite this, it is necessary to clear one’s mind of what is seen as typical behavior for men and to encourage the men in your lives to seek mental health help. By overcoming stigmas, more men can receive the necessary help for BPD and live fulfilling lives.

Personality disorders can often go unnoticed. This is because they are typically difficult to spot and hard to treat. It is very easy to dismiss or look past one’s behavior and simply label it as their personality, never looking more into it. Since it is so difficult to spot, it is important for these individuals to seek help for themselves when the symptoms may be atypical or cause distress.

Borderline personality disorder, also known as BPD, is a mental health disorder characterized by instability and impulsivity. This instability and impulsivity often makes it difficult to function in everyday life.

Symptoms of BPD:

  • Efforts to avoid real or imagined abandonment
  • A pattern of unstable and intense interpersonal relationships
  • Persistent and unstable sense of self
  • Impulsive behaviors
  • Self-harming behavior
  • Rapidly changing mood swings
  • Feeling like they are “losing touch with reality”
  • Difficulty regulating emotional reactions

About 2% of adults are affected by BPD and 75% of those affected are women. Luckily, considerable amounts of research on BPD have been done over the years, allowing us to further understand and assist those struggling with BPD. Borderline Personality Disorder: Helping Yourself and Your Family

Borderline personality disorder (BPD) is a Cluster B personality disorder. Like other disorders in this category, including histrionic, narcissistic, and antisocial personality disorder, BPD is characterized by unpredictable, dramatic, and overly-emotional behavior and thoughts. Specific to borderline patients is a persistent fear of abandonment by others, which leads to unstable relationships, suicidal gestures, self-damaging behavior, anger and emptiness, and even dissociative symptoms. For those with this condition, their symptoms not only interfere with their daily functioning but can also strain their closest personal relationships. Having a family member that suffers from a personality disorder can be a difficult and distressing experience. When it comes to coping with borderline personality disorder in family members, steps can be taken to help both them and yourself.

First of all, it is important to remember that it is not your job to treat and constantly provide reassurance for a borderline family member. You can still show that you love and care about them, but not to the point that it affects your own mental health. It may be difficult, but you should not put your own life on hold for someone else or be their punching bag, verbally or physically. At that point, it is imperative to step away from the situation and allow the person to seek help on their own. Professional help is available to them in the form of medication and therapy.

Although there are no BPD-specific drugs, medications can be used if the patient is experiencing symptoms of other disorders with their BPD. For example, some people with borderline personality disorder experience depressive episodes so they may be prescribed antidepressants, such as escitalopram or fluoxetine. Mood-stabilizers can also be used and if a patient also has psychotic features, they may be treated with antipsychotics.

Besides medication, there are also multiple types of therapy that can be used to treat borderline personality disorder. Dialectical behavior therapy is a treatment that is available and is used to treat BPD specifically. Through this therapy, patients work on their ability to cope with their intense emotional responses and learn the skills needed to deal with crises and other distress in a healthy manner. It is easy to be overwhelmed when faced with borderline personality disorder in family members, but help is out there.

Anxiety, Depression, Eating Disorders, ADHD, Et al: How to Support a Friend with Mental Illness

When helping a friend with a mental illness, the first step should be assessment of their symptoms. Sometimes they just might be going through a difficult time, but if certain common symptoms associated with mental health issues persist it is imperative to respond sensitively. Majority of the time, friends will just want to know they have your support and that you care about them. A good way to show your support is by talking to them. If you provide a non-judgmental space for them to speak about their issues it will help encourage them to be open with their problems. Let them lead the conversation and don’t pressure them to reveal information. It can be incredibly difficult and painful to speak about these issues and they might not be ready to share everything. If you aren’t their therapist do not diagnose them or make assumptions about how they are feeling, just listen and show you understand. If someone doesn’t want to speak with you, don’t take it personally, just continue to show them you care about their wellbeing and want to help as much as possible. Just knowing they have support can give them the strength they need to contact someone who can help them.

If a friend is having a crisis, such as a panic attack or suicidal thoughts, you must stay calm. Try not to overwhelm them by asking a lot of questions and confronting them in a public setting. Ask them gently what would be helpful to them right now or reassure them. If they hurt themselves, get first aid as soon as possible. If someone is suicidal, contact the suicide hotline at 800-237-8255 immediately.

The best way to help someone is by connecting them to professional help. By expressing your concern and support you can show them that they can get help and their mental health problems can be treated.

Mental Illness in Young Adults

You may think you’re all grown up, you might even be the tallest you’ll ever be at this point in your life but young adults, ages 18 to 29, are still experiencing cognitive development. Brain development does not stop once you reach 18. In fact, the cognitive growth process is only half way done. Parental controls, attending college, changes and experiences that happen to young adults in their early twenties can affect and significantly shape brain development. This is why mental illnesses can present at this time in life. 

Mental Illness envelops a wide assortment of issues that exist on a severity continuum. Some can be brief or temporary reactions to emergency or different encounters, while others are chronic conditions. Mental Illnesses have different causes and triggers and present in different ways.

Mental Illnesses that Commonly Present in Young Adults: 

  • Eating Disorders (anorexia,bulimia, bingeing) 
  • Addiction (opioids, tobacco, alcohol) 
  • Anxiety Disorders (social anxiety, generalized anxiety, phobias) 
  • Personality Disorders (antisocial, borderline personality disorder) 
  • Mood Disorders (bipolar disorders, major depressive disorder) 
  • Thought Disorders (schizophrenia)

Young adults are at a particularly vulnerable time in their development, which might give reason as to why 1 of every 5 is affected by mental illness. Given the right conditions, stress can trigger mental illness. 

Considering young adults’ brains are still developing, diagnosing mental illness and treating or managing it can improve chances of a great outcome. Most young adults with mental illness can learn to successfully manage symptoms and live happy lives with the right help.

Oftentimes when people say “I’m so OCD,” what they really mean is “I’m a perfectionist.” Perfectionism is a trait characterized by a rigid need to be perfect: that is, without flaws. Although perfectionism is often thought of as being a positive trait, it can be maladaptive in many ways. Perfectionists tend to have unrealistically high standards, to take longer to complete tasks, to struggle with collaboration, and to place immense pressure on themselves. When perfectionism is extreme, it may meet criteria for Obsessive-Compulsive Personality Disorder (OCPD).

OCPD, not to be confused with OCD, is a personality disorder characterized by a pattern of inflexibility and a fixation with perfection. This fixation can interfere with one’s ability to complete tasks, one’s interpersonal relationships, and other aspects of one’s life. People with OCPD attempt to control life using details, rules, lists, order, organization, and/or schedules. According to the Diagnostic and Statistical Manual of Mental Disorders, OCPD is diagnosed when an individual demonstrates:

  • a fixation with details, rules, lists, order, organization, or schedules
  • perfectionism that interferes with task completion
  • overemphasis on work and productivity, leaving no time for leisure or relaxation
  • a fixation with morality and ethical responsibility
  • hoarding of objects and/or money
  • an inability to delegate tasks to others
  • an overall rigidity and/or stubbornness

Like other psychological disorders, OCPD can be treated with therapy and medication. Therapies for OCPD emphasize identifying and modifying maladaptive behavioral patterns, as well as targeting triggers (such as stress) for overly perfectionistic behavior.

You may have seen quizzes online that can help determine your personality. Most of the quizzes online revolve around the basis of five core personality traits. Fiske, Norman, Smith, Goldberg, and McCrae & Costa were the leading researchers that brought evidence of the big five traits. The five traits are scaled on a spectrum, for example if a person was rated low in Neuroticism; they were rated high in Emotional Stability. The five traits are categorized as:

  • Openness: high levels of imagination, insight, tend to be adventurous, creative
  • Conscientiousness: high levels of thoughtfulness, goal-directed behaviors, good impulse controls, and organized
  • Extroversion: high levels of excitability, sociability, talkativeness, assertiveness, and high amounts of emotional expressiveness.
  • Agreeableness: high levels of trust, altruism, kindness, affection, and other prosocial behaviors
  • Neuroticism: high levels of sadness, moodiness, and emotional instability. They tend not to handle stress well.

These five traits have been found to be universal. One study showed that people in more than 50 different cultures found that the five dimensions could be accurately used to describe personality. Also, the five dimensions have biological and environmental origins that can influence the change of personality.

Another study showed that our five factors change over time. It showed that agreeableness and conscientiousness increased, but extroversion, neuroticism, and openness generally decrease as a person ages. Sex also contributes to the five factors as well. Women tend to score higher in both agreeableness and neuroticism. Even though sex differences have been found, it does not, by itself, demonstrate that the sexes are innately different in personality, although that is a possibility.

Frank Sulloway, a psychologist who focused on birth order, found that personality traits correlate with the order of individuals’ birth. He found that firstborns are statistically more conscientious, more socially dominant, less agreeable, and less open to new ideas compared to those born later. This could be due to firstborns caring for their younger siblings at a young age.

The Big Five is not based on any underlying theory; it is merely an empirical finding, meaning that the underlying causes behind them are unknown.

We all know someone who prefers to stay in rather than go out and party or someone who barely talk in a group setting. We may call them shy, quiet, or maybe socially awkward. But they may just be an introvert. Introversion and shyness are often times used together. However, shyness revolves around the fear of negative judgment while introversion is the preference for quiet, minimally stimulating environments. So it is possible for a person to be a shy extrovert, where the individual is afraid to speak up, fearing negative judgment, more so than exhausted in a certain social situation.

Despite the difference, there is also an overlap between shyness and introversion, e.i. many shy people are introverted. Some people are born with “high-reactive” temperaments that predispose them to both shyness and introversion. A shy person may become more introverted over time, motivated to discover the pleasures of solitude, other minimally stimulating social environments, and to move away from judgments. On the other hand, an introvert may become shy after continually receiving the message that there’s something wrong with them.

There’s a shared bias in our society against both shyness and introversion. Neither trait is welcomed in our society because studies have shown that we rank the fast and frequent talkers as more competent, likeable, and even smarter than slow and quiet talkers.

Here are 5 ways introverts can spend time that is deeply fulfilling and socially connected:

  1. Reading. Books transcend time and place. Studies have shown that reading fiction increases empathy and social skills.
  2. Enter a state of “flow” by doing work or a hobby that you love. Flow is the transcendent state of being, in which you feel totally engaged in an activity. People in flow don’t tend to wear the broad smiles of enthusiasm. When you watch them in action, the words “joy” and “excitement” don’t come to mind. But the words “engagement,” “absorption,” and “curiosity” do.
  3. Keep an informal quota system of how many times per week/month/year you plan to go out to social events and how often you get to stay home. This way you can plan which parties or get-togethers you can truly enjoy and which you don’t. So you are less likely to drive yourself mad thinking you should’ve stayed home.
  4. Have meaningful conversations.
  5. Spend time and show affection to the ones you love, whose company is so dear and comfortable that you feel neither over-stimulated nor anxious in their presence.

It is common in today’s world to call someone who is very clean “OCD”, or someone who is very active “ADHD”, or someone who is overly confident a “narcissist”.  What many people do not know is that narcissism is not an adjective to describe someone’s personality, but a real psychiatric diagnosis. The DSM IV identifies narcissism as a personality disorder. Personality disorders are characterized by a set of rigid traits, thoughts, and behaviors that are unhealthy and inflexible. Narcissistic personality disorder or (NPD) is characterized by an overinflated sense of self, preoccupation with personal success, and apathy for other’s emotions.

Signs of narcissistic personality disorder:

  • Grandiose sense of self-importance
  • Preoccupation with fantasies of success, power, brilliance, beauty.
  • Belief that one is unusually special or unique
  • Need for excessive admiration
  • Strong sense of entitlement
  • Exploitative of others
  • Lacks empathy
  • Is often envious of others or believes that others are envious of them
  • Frequent display of arrogant or haughty behavior

Although people with NPD display an arrogant and larger-than-life picture to the outside world, they can also suffer from low self-esteem internally. The constant need for approval and obsession with success can be stressful because in reality, one will never gain approval from everyone and one will never achieve everything they want.  Trouble with interpersonal relationships often results from the exploitative and apathetic behaviors that people with NPD believe will help them achieve their goals.

People with NPD can look like the perfect partner upon first meeting. They are often charismatic and appear to be very put together. However, being in a relationship with a person with NPD can be complicated and stressful. People with NPD lie frequently to get what they want, which can cause a breach of trust in a serious relationship. People with NPD rarely apologize because they lack the empathy to understand the point of view of their partners. Additionally, people with NPD think that they are perfect and will dismiss others who have opinions contrary to theirs. Although people with NPD can be extremely difficult to live with, they can still be a loved one that you care about.

Bulimia Nervosa is an eating disorder known for episodes of out of control eating and subsequent self-induced methods of purging in order to prevent weight gain. Binging is considered an irresistible compulsion and on average happens about once a week, but can also occur several times a week. Bulimia is most common in women and typically begins in their teens to early 20s. The cultural obsession with a perfect body image, fueled by social media models with unattainable body “goals”, is harmful to young people’s self-esteem and body image. Social media perpetuates a cultural and social expectation of women needing to be thin in order to be beautiful, and this negatively impacts our younger generations.

Signs of Bulimia include:

  • Yellow teeth, erosion of dental enamel
  • Gastrointestinal problems
  • Unexpected weight gain
  • Hair loss
  • Lack of bowel movements
  • Lack of energy
  • Dehydration
  • Red, blistered, or scabbed knuckles

Some typical purging methods include:

  • Vomiting
  • Taking laxatives or diuretics
  • Excessive exercise

Some typical restrictive eating symptoms include:

  • Fasting
  • Disordered Eating

Research has shown that people with Bulimia Nervosa have comorbid mental illnesses such as anxiety, depression, or substance use disorder. Bulimia is also related to personality disorders which effect one’s social, personal, behavioral, and emotional interactions. Bulimia is best treated through a primary care physician, a nutritionist, and a mental health clinician. Cognitive behavioral therapy as well as antidepressants are extremely helpful for those recovering from Bulimia. It is important that schools address eating disorders at an early age in order to educate and bring awareness to young people.
